Skilled Nursing Services (Private Duty Nursing – PDN)
SSS3 provides high-acuity, continuous skilled nursing care for medically fragile pediatric and adult patients in the home setting. Services are delivered by licensed Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) under physician-directed plans of care.
Includes:

Ventilator and tracheostomy care

G-tube/NG tube feeding and management

Medication administration and IV therapy

Seizure monitoring and intervention

Therapy Services (Rehabilitative Care)
SSS3 coordinates and delivers rehabilitative therapy services designed to improve mobility, communication, independence, and overall functional outcomes.
Includes:

SSS3 Protective Care Nursing & Therapy Services supports patients and families by providing and coordinating access to medically necessary durable medical equipment and clinical supplies that promote safe, effective, and independent care in the home environment.
As part of our coordinated care model, we work closely with patients, caregivers, physicians, therapists, and equipment providers to identify, furnish, and facilitate access to resources that support mobility, respiratory health, feeding needs, safety, and overall clinical outcomes. Our goal is to ensure patients have the equipment and support necessary to safely manage their healthcare needs while remaining in the comfort of their homes.
