Skilled Nursing Services (Private Duty Nursing – PDN)

SSS3 provides high-acuity, continuous skilled nursing care for medically fragile pediatric and adult patients in the home setting. Services are delivered by licensed Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) under physician-directed plans of care.

• Ventilator and tracheostomy care

• G-tube/NG tube feeding and management

• Medication administration and IV therapy

• Seizure monitoring and intervention

• Chronic condition management (e.g., neurological, respiratory)

• Post-hospitalization stabilization

Therapy Services (Rehabilitative Care)

SSS3 coordinates and delivers rehabilitative therapy services designed to improve mobility, communication, independence, and overall functional outcomes.

Includes:

​

• Physical Therapy (PT): mobility, strength, balance

• Occupational Therapy (OT): daily living skills, fine motor function

• Speech-Language Pathology (SLP): communication and swallowing disorders

• Respiratory Therapy (if applicable later)

• Audiology (optional expansion)

Personal Care & Support Services

Non-medical assistance provided to support daily living activities while maintaining patient dignity and safety.

Includes:

• Bathing, grooming, dressing

• Mobility assistance and transfers

• Meal preparation and feeding support

• Light housekeeping related to patient care

• Companionship (supportive—not positioned as primary service)

Care Coordination & Case Management

SSS3 ensures seamless coordination between patients, families, physicians, therapists, and payers to optimize outcomes and continuity of care.

• Plan of care development and updates

• Coordination with physicians and specialists

• Hospital discharge planning support

• Referrals and service integration

• Family education and support

Pediatric Specialty Care

A dedicated service line focused on medically complex children requiring specialized, compassionate, and family-centered care in the home.

Includes:

• Medically fragile pediatric care (vent/trach dependent)

• Developmental support integration

• School coordination (IEP/medical support)

• Family caregiver training and education

Maternal Support Services (Doula Services)

Non-clinical maternal support services designed to provide emotional, educational, and physical support before, during, and after childbirth.

• Emotional wellness support
